Preparing for Labour: What the Evidence Supports

Pre-birth acupuncture is one of the more commonly requested things in a Chinese medicine clinic, and one where the evidence is genuinely worth reading rather than assuming.

Ripening, softening and settling

Traditional preparation aims to have the body ready rather than to force anything: qi and blood moving freely, the lower back and pelvis relaxed, and the mother settled rather than braced. Where there is exhaustion late in pregnancy, treatment nourishes; where there is anxiety and tension, it calms; where there is fluid retention and heaviness, it moves. The intent is readiness, not induction — a distinction we are careful about.

The Systematic Classic includes a short entry on labour itself: for females in whom delivery is difficult or in whom the placenta is retained, Kun Lun Mountain (Kun Lun, BL-60) is the ruling point. It is a sparse line and it should be read as one — the classics record the point and the indication and claim nothing about the outcome. That is worth keeping in view alongside the modern trial evidence, which is genuinely mixed and is discussed below rather than skated over. Nothing here replaces the plan you have made with your midwife or obstetrician.

What pre-birth treatment involves

Pre-birth treatment usually begins around 36 to 37 weeks with weekly appointments until birth, using a well-established point protocol. Many women also learn acupressure points that a partner can use during labour itself, which is often the part people find most valuable — something for the support person to do that genuinely helps. Everything is coordinated with your midwife or obstetrician.

What the 2020 Cochrane review found

The 2020 Cochrane review of acupuncture or acupressure for pain management during labour (Smith and colleagues) included 28 trials and 3,960 women. It found that acupuncture may increase satisfaction with pain relief compared with sham and probably reduces the use of pharmacological analgesia, while rating certainty as low to very low for several other outcomes; acupressure probably reduces the need for caesarean section compared with sham control (doi:10.1002/14651858.CD009232.pub2). That is a real, sizeable review with a mixed but not empty result — and reporting both halves of it is the honest way to present it.

How we work with your maternity team

Women booking pre-birth appointments at Balmoral and Alexandra Hills are usually planning ahead, which makes this some of the calmer work we do. We teach the partner the acupressure points, because labour is where they get used and the support person is usually looking for something practical to offer. Your maternity team leads; we prepare.

The last few weeks

Keep moving — walking and upright positions help the baby settle well. Practise the acupressure points with your partner before labour so they are familiar rather than improvised. Rest properly in the last weeks; arriving at labour exhausted helps nobody. Know your options and preferences, and know that plans change. Ask your midwife about anything you are considering, including this.

When to go straight to the hospital

Go to hospital or call your maternity team immediately for reduced fetal movements, bleeding, waters breaking, severe headache with visual changes or upper abdominal pain, or contractions before 37 weeks. Nothing on this page is a reason to delay contacting them — pre-birth treatment sits entirely within, and behind, your maternity care.

Can acupuncture bring on labour?

We describe this work as preparation rather than induction, and we would not promise to start labour. Medical induction is a decision for you and your maternity team, made on clinical grounds.

When should we start?

Usually around 36 to 37 weeks, weekly until birth. That timing lets the preparation build and gives your partner time to practise the acupressure points properly.

What can my partner actually do during labour?

We teach specific acupressure points for the lower back and hands that partners can use during contractions. Many couples say it was the most useful thing they took from the appointments — a practical role rather than standing by.
