Carpal Tunnel Syndrome: Numb Hands That Wake You

Carpal tunnel syndrome wakes you at night shaking your hand, drops things you meant to hold, and turns fiddly tasks into frustrations. It is also a mechanical problem, and honesty about that shapes what treatment can offer.

Obstruction at the wrist

Chinese medicine reads it as obstruction of qi and blood in the channels crossing the wrist — the Pericardium channel in particular runs directly through the region. Cold-damp obstruction gives the aching, stiff, weather-sensitive presentation; blood stasis the fixed, numb, night-waking picture. Underlying blood deficiency, poorly nourishing the sinews, is common where symptoms are bilateral or long-standing. Pregnancy, thyroid disease and diabetes all contribute and all need identifying.

The Systematic Classic groups the hand symptoms in a way that reads oddly close to the modern description. Among the signs ruled by Lesser Shang (Shao Shang, LU-11) it lists insensitivity of the hands and arms, cramp of the wrists, pain in the finger joints, and what it calls finger bi — bi being blockage, the same term used for painful obstruction elsewhere. Numbness, wrist cramp and finger pain recorded as one presentation, with the problem named as something blocked at the wrist rather than a fault in the fingers, is a fair description of what a compressed nerve does.

What wrist treatment involves

Treatment needles local points at the wrist and forearm, often with electroacupuncture, alongside points for the underlying pattern. We also look upstream at the neck and forearm, since symptoms attributed to the wrist sometimes originate higher. Splinting at night is frequently the most useful single measure and we will teach it. Where there is real weakness or wasting of the thumb muscles, our advice is a surgical opinion, promptly.

What the Cochrane review concluded

The Cochrane review of acupuncture and related interventions for carpal tunnel syndrome (Choi and colleagues, 2018, 12 studies, 869 participants) concluded that effectiveness remains uncertain, with most studies at unclear or high risk of bias, and made a point worth repeating: acupuncture cannot be expected to relieve the mechanical and structural problem, and is appropriate as an adjunct or in mild symptoms rather than as a substitute for definitive treatment (doi:10.1002/14651858.CD011215.pub2). We agree with that framing and work within it.

How we approach hand numbness here

Hands arriving at Balmoral and Alexandra Hills are often those of pregnant women, keyboard workers and tradies. The first thing we establish is severity: mild, intermittent, night-predominant symptoms are reasonable to treat conservatively; constant numbness, weakness or muscle wasting is a different conversation and belongs with a surgeon. Pregnancy-related carpal tunnel usually settles after birth, which changes the calculation.

Protecting the wrist day and night

Wear a neutral wrist splint at night — this is the single most useful measure for most people, and the wrist bending during sleep is what wakes you. Adjust your workstation so wrists are not extended while typing. Take breaks from sustained gripping and vibrating tools. Shake and stretch the hand through the day. Get thyroid and glucose checked if you have not, since both contribute. And do not ignore weakness — that is the sign that changes the plan.

Symptoms that need a surgical opinion

Constant numbness, weakness of grip, or visible wasting at the base of the thumb means nerve damage may be progressing — get a surgical opinion promptly, as delayed decompression can leave permanent deficit. Sudden severe hand pain, swelling or colour change needs urgent assessment. Numbness affecting the whole hand or arm suggests the problem may be higher up.

Can acupuncture avoid surgery?

For mild, intermittent symptoms it may be a reasonable part of conservative management. For significant weakness or wasting, surgery is the treatment that prevents permanent nerve damage, and we will send you for an opinion rather than treat around it.

Is the night splint really necessary?

It is often the most effective single measure, because the wrist bends during sleep and that is what wakes you. A neutral splint costs little and works for many people.

Mine started in pregnancy — will it go?

Pregnancy-related carpal tunnel commonly settles after birth as fluid retention resolves. Night splinting and conservative care usually carry you through, and we treat with the usual pregnancy point precautions.
